AI in healthcare: adoption rises, evidence lags

Artificial intelligence has firmly established itself within modern healthcare. From clinical documentation to diagnostics and predictive analytics, AI-driven tools are increasingly embedded in hospital workflows. Yet despite their rapid adoption and demonstrated technical accuracy, a fundamental question remains unanswered: do these technologies actually improve patient outcomes?

Jenna Wiens and Anna Goldenberg recently highlighted this concern in a publication in Nature Medicine. According to Wiens, the shift toward AI adoption in healthcare has been swift and, in some cases, insufficiently scrutinized.

Adoption outpaces evaluation

For years, AI researchers struggled to convince clinicians of the value of machine learning in medicine. That dynamic has changed dramatically. Healthcare providers are now not only receptive to AI tools but are deploying them at scale. However, the speed of implementation appears to have outpaced rigorous evaluation.

A 2025 study led by Paige Nong at the University of Minnesota found that approximately 65 percent of U.S. hospitals had adopted AI-assisted predictive tools. Notably, only two-thirds of those institutions assessed the accuracy of these systems, and even fewer examined potential bias. The gap between use and validation raises questions about oversight and accountability.

Accuracy is not the same as impact

AI systems in healthcare are often praised for their ability to process large datasets and identify patterns with high accuracy. Applications range from interpreting X-rays to flagging patients at risk of deterioration. However, accuracy alone does not guarantee clinical benefit.

An AI tool might, for instance, rapidly analyze a chest scan. But its true value depends on how clinicians interpret and integrate that information into decision-making. Will doctors rely on AI recommendations? Will these tools subtly influence treatment choices or patient interactions? And crucially, will they lead to better health outcomes?

These questions remain largely unexplored. Much of the existing research focuses on technical performance or user satisfaction rather than measurable improvements in patient health. As Wiens notes, studies often evaluate whether clinicians find AI tools helpful, but not whether patients ultimately benefit.

Unintended consequences

One of the most widely adopted applications is so-called “ambient AI,” also known as AI scribes. These systems listen to doctor–patient conversations and automatically generate clinical notes. Early evidence suggests they can reduce administrative burden and alleviate clinician burnout. This is an important consideration in overstretched healthcare systems.

Anecdotal reports from hospitals indicate that physicians appreciate the ability to focus more fully on patient interactions rather than documentation. However, the downstream effects on clinical care are less clear.

There is growing concern that reliance on such tools could alter how clinicians process information. Research from other domains suggests that automation can influence cognitive engagement. In a medical context, this raises important questions: could AI tools change how doctors interpret patient data? Might they affect the training of medical students, shaping how future clinicians think and make decisions?

These potential unintended consequences underscore the need for a more nuanced understanding of AI’s role. Efficiency gains, while valuable, should not come at the expense of clinical reasoning or patient safety.

Improving healthcare delivery

Despite these uncertainties, experts do not advocate slowing innovation altogether. Wiens, for example, emphasizes that AI holds significant promise for improving healthcare delivery. The challenge lies in ensuring that its implementation is guided by robust evidence rather than assumption.

Future research must move beyond technical validation to examine real-world impact. This includes assessing how AI tools influence clinical workflows, decision-making processes, and ultimately patient outcomes. It also requires greater attention to context: the effectiveness of a tool may vary between hospitals, departments, and individual practitioners.

The path forward is unlikely to be binary. As Wiens suggests, the future of healthcare will not be defined by choosing between AI or no AI, but by finding a balanced integration. Achieving that balance will depend on careful evaluation, transparency, and a willingness to question not just what AI can do, but what it should do.

For now, the healthcare sector stands at a critical juncture. The technology is advancing rapidly, but the evidence needed to justify its widespread use is still catching up.
